Position: Personal Advisor
Location: Delamere Street, CW1 2JZ
Start Date: ASAP
Contract Duration: 6+ Months
Working Hours: 36 hours per week
Pay Rate: £ 15.84 Per Hour

Job Responsibilities
Provide guidance and support to young people transitioning from care to independent living
Develop, review, and maintain individual pathway plans
Carry out assessments to identify needs and appropriate support services
Work closely with partner agencies to coordinate effective support
Support safeguarding processes and escalate concerns where necessary
Maintain accurate, timely case records in line with policy requirements
Promote independence, wellbeing, and positive outcomes for service users
Ensure compliance with statutory duties and local authority procedures
Person Specification
Must-Have Requirements
Relevant qualification in social care, health, or related field
Experience working in social care or supporting vulnerable young people
Strong understanding of safeguarding principles and procedures
Ability to manage a caseload and maintain accurate records
Excellent communication and interpersonal skills
Enhanced DBS clearance (or willingness to obtain)
Right to work in the UK
Nice-to-Have Requirements
Experience working within a local authority setting
Knowledge of leaving care services or similar support frameworks
Familiarity with pathway planning and statutory guidance
Experience working with multi-agency teams
